You may want to print out these instructions if you are planning on paying by credit card.
- Print out the Summer Luncheon Registration Form, fill it out and mail / fax it to Mary Jo Durivage
- Pay by credit card (Visa, MasterCard, American Express, and Discover). Select your registration option below.
- Click on the appropriate button below, scroll to the bottom of the web page, look for "Don't Have a PayPal Account?", and click on "Continue". You do not need a PayPal Account. You do not need to register or login to PayPal. If you already have a PayPal Account, you have the option of using it.
- Fill out your name, billing address, and credit card information.
- Click on "Review Order and Continue".
- You can print out your order at the end of the process, along with your confirmation number.
- You will also receive an email confirmation immediately after your transaction.
