You have two options for paying for your MDMLG Membership: check and credit card. Regardless of which payment method you select, you must fill out the Membership Invoice and mail / fax to Audrey Bondar.

• If you are paying by check, fill out the Membership Invoice and mail to Audrey Bondar along with your check.
